We are collecting the data through a google form (part of google docs)
parsing the data into a separate spreadsheet to hide the email addresses, (since this is a public site and we don't feel comfortable disclosing that kind of info without permission), timestamps etc.
embedding the spreadsheed in an iframe as a widget ('other HTML')
